Rates and In-Network Insurance in Oregon and California
Celium Healthcare is in network in Oregon with Regence BlueCross BlueShield, Moda, UnitedHealthcare, First Choice Health (including Adventist and Samaritan plans), and ComPsych, and in California with Anthem Blue Cross, UnitedHealthcare, and ComPsych. Most out-of-state Blue plans work through BlueCard. Self-pay is $550 for the intake and $325 for a follow-up.

Existing California patients may continue to use Lyra, Aetna, and Cigna.
Not sure whether your plan is in network? Go ahead and book. We'll verify your insurance before your visit, and if NP Lee is out of network we cancel at no charge.
High-deductible plans. Until your deductible is met, you pay the insurer's contracted rate, roughly $500 to $650 for an intake and $325 to $400 for a 20-minute follow-up depending on the plan. We'll tell you the exact figure when we verify your benefits.
Other PPOs. Out of network; you receive a detailed receipt to submit for reimbursement.

Payment
Most of my clients pay a small co-pay or co-insurance fee per visit. Payments are due at the time of your visit.
Private-Pay or Self-Pay
Initial Evaluation (about an hour), or a 45-minute Ongoing Session: $550
20-minute Ongoing Session: $325
Telephone & Text Messages
No fee for up to five brief calls and text messages per month. Maximum 5 minutes each.
Calls/texts that exceed 5 minutes and/or beyond five brief calls/text messages per month will be prorated at the hourly rate of $500.
Form Fee Guidelines
Any letters, forms, or documents that you need me to complete are done outside of scheduled appointment times and may be subject to an administrative fee.
These may include prior authorization appeals for medications not on your insurance formulary.
Payment is due in advance.
Fees start at $15 minimum and can increase depending on the complexity and time involved to complete.
