Thank you for your interest in making an appointment with Erik Lee, PMHNP.

Erik provides clinical services exclusively through his solo practice, Celium Healthcare.

If you are currently in crisis or having suicidal thoughts, please do not schedule.

For medical and psychiatric emergencies, call 9-1-1 or go to the nearest emergency room.

Criteria for Self-Scheduling

Important! Please read before scheduling!

  • You are age 18 or older.

    AND you are a resident of Oregon

    *OR you are a resident of California with UnitedHealthcare PPO (only).

    We apologize - we do not accept other insurance plans in California.

  • You are willingly and voluntarily making this appointment for yourself. 

    Please do not make an appointment for someone else.

  • You have commercial (non-OHP, non-Medicare) insurance:

    • Adventist Health (under FCHN),

    • Blue Cross Blue Shield (Most plans, incl. Anthem) (Oregon only),

    • First Choice Health Network (FCHN),

    • Moda,

    • Regence BCBS (Oregon only),

    • UnitedHealthcare (Oregon and California), or

    • Samaritan Health (under FCHN).

    • ComPsych

    Feel free to schedule while we verify your insurance. Should NP Lee be out-of-network, we can cancel the appointment without penalty.

  • You understand that we require a valid credit card on file for out-of-pocket expenses, such deductibles, copay, coinsurance, no show, and late cancellation fees.

  • You understand that NP Lee sees patients for these issues (and may or may not be able to address other issues):

    • Alcohol / Substance use / Addictive behavioral issues

    • Attention-deficit/hyperactivity disorder (ADD/ADHD)

    • Anxiety (panics, social anxiety, generalized anxiety)

    • Bipolar disorder or mood instability

    • Binge eating

    • Depression, sadness, not finding joy

    • Insomnia, sleep disturbance

    • Medications for Opioid Use Disorder (MOUD)

    • Obsessive compulsive thoughts and / or behaviors

    • Post-traumatic stress

    • Side effects of medications, and/or problems with withdrawals

  • NP Lee mainly offers diagnostic evaluation and/or medication management services. Brief psychotherapy may be offered only in addition to these services.

    NP Lee is not accepting new patients solely seeking psychotherapy. 

  • NP Lee does not support disability or leave applications for new patients. 

  • You are willing to see NP Lee mainly via telemedicine.

    You are ALSO willing to come for rare in-person evaluations at the office, if requested. Note: Such requests would be rare.

  • Please note that while NP Lee may prescribe benzodiazepines and "z-medications," he ONLY does so for short-term, judicious as-needed use; and NOT for daily use (unless as part of a strict taper protocol.)

    Benzodiazepines are anti-anxiety medications that usually end in -azolam or -azepam). (Click here for examples).

    "Z-medications" are benzodiazepine receptor agonists (BZRAs) used as hypnotic/sleep medications.

If after reading the above, you are ready to make an appointment, please prepare:  

  1. E-copy of photo ID (front and back);

  2. E-copy of insurance card (front and back), and know the date of birth of the policy holder, if that is not you;

  3. Credit or debit card, or FSA or HSA card; and
