Why the DIVA 5 Is the Gold Standard for Adult ADHD Diagnosis

Accurately diagnosing ADHD in adults can be challenging. Symptoms often overlap with anxiety, depression, trauma, and other conditions, making it essential to use validated diagnostic tools rather than relying on self-report alone. At Celium Healthcare, I typically use the Diagnostic Interview for ADHD in Adults (DIVA-5), a structured interview recognized internationally as a reliable foundation for diagnosis.

Why DIVA-5 Matters

The DIVA-5 is based on the criteria in the DSM-5-TR and carefully explores both childhood and adult symptoms of ADHD across multiple life domains. Unlike quick questionnaires or symptom checklists, the DIVA provides a structured framework that ensures important details are not overlooked. It also gives patients space to reflect on how symptoms have affected different parts of their life, from school and work to relationships and daily functioning.

What the Research Shows

The validity of the DIVA has been supported in peer-reviewed studies. In one study of 108 patients, 60 with ADHD, researchers compared the DIVA 2.0, the Adult ADHD Self-Report Scale (ASRS), and eight neuropsychological tests. Results showed that:

  • Most tools had poor ability to distinguish ADHD from non-ADHD.

  • The DIVA 2.0 stood out with relatively strong discriminative power (sensitivity = 90%, specificity = 72.9%).

  • When combined with results from a continuous performance test, specificity improved by about 10%, further strengthening diagnostic accuracy.

How I Use the DIVA-5 in Practice

At Celium Healthcare, I integrate the DIVA-5 into a broader evaluation that includes:

  • A comprehensive clinical interview

  • Review of developmental, academic, and psychiatric history

  • Input from collateral sources when possible

  • Screening for co-occurring conditions (such as anxiety, depression, bipolar disorder, or trauma)

This structured approach helps avoid misdiagnosis and ensures that treatment planning is grounded in evidence.

Moving Beyond Checklists

ADHD is often oversimplified by short quizzes or informal assessments online. While these can raise awareness, they are not a substitute for a full diagnostic interview. The DIVA-5 provides a more reliable path to clarity, helping patients feel understood and giving providers a strong foundation for clinical decisions.

Conclusion

The DIVA-5 remains one of the most robust tools available for adult ADHD diagnosis, offering high sensitivity and good specificity. When paired with additional cognitive testing and careful clinical judgment, it increases confidence in diagnosis and guides effective treatment planning.

At Celium Healthcare, my goal is to bring this level of rigor and care to every patient I work with, ensuring that ADHD is identified accurately and managed thoughtfully.
